NEW SCCA Medical Forms

Post by VLVTLVS » Sun Feb 21, 2016 7:48 pm

IMPORTANT FYI -

I recently spoke to Kathy Cowen, SCCA Member Services, and she informed me that the SCCA Med. Form, that I downloaded from the WHRRI web forms, is NOT Current.

She sent me the most current version.

If they fill out the old version, it will be denied.

Not sure who I should send the PDF to, in order for WHRRI Members to fill out the correct version.

I've attached the most current version - hope it works.

"If they fill out the old version, it will be denied"
What if you are using old form because you are only doing a renewal? Will it be denied?
"Renewals:
Applicants that are less than 40 years old must renew their Physical Examination every five years.
Applicants that are at least 40 years old must renew their Physical Examination every three years"
Thanks

Great. So the exam I had last month is now junk??!? Mind you, it wasn't EVEN POSSIBLE to find the form on the SCCA site, so I had to go to our site to find a copy...

GREAT.

I guess I'll be calling Kathy this morning and requesting them to get their $$## together for the application that's already in the mail...

PS: PDF worked fine for me, Mike... thanks!

Talked to Kathy, she was very helpful... she clarified that the real area of concern with new vs. old form will be if there is a need for a medical review; if your doctor has signed off without reservations to recommend you for racing, you should be OK.

We also talked about being unable to find the form; she gave me the guided tour, it's buried under "About SCCA" "Downloads". So here's the link to the page:
http://www.scca.com/downloads

...and the link directly to the form itself:
http://www.scca.com/downloads/2800-phys ... e/download

I've also sent an email to the web support email, based on her recommendation, pointing out how many times they've failed to make the form accessible on the site, in the hope that they'll fix it. It's not that hard!

I'd also encourage anyone else here reading this that's an SCCA member to provide the same feedback. They've put a lot of time and money into making the website look pretty and up-to-date... but it's GOT to be able to provide basic function of a '90's-era web page, or it's completely useless... </rant>

I will update our site to link to the SCCA downloads page instead of caching the form locally. one more click, but I like that better than providing bad info...

If one is SCCA member all the forms are relatively easily accessible through ams.scca.com under `Forms'. There are also other interesting documents in `File Cabinet' under `Resources'.

The place to find the correct form is: http://www.scca.com/downloads/2800-phys ... e/download. You do not have to be an SCCA member to access this link.

I will let SCCA know that the link on their member page is incorrect.

You may also access the current form on the MSR Drivers School registration page.

Is there any recommended medical clinics in the Livonia area that will do the SCCA physical? I am having a very hard time finding a place that is willing. Even my regular Dr. will not do it. They are all concerned they will be liable for something. I'm in good health and I am turned away as soon as they hear RACE CAR. I always had a hard time finding a place in the past but did find one, now they are out of business.

Any help would be great.

In the OLD forum I seem to recall a post for a physician in Wixom that would do Waterford and SCCA physicals but I believe that is long lost information. Andrew, is there anyway to find it?

Doug, Ann Arbor's probably not what you're looking for, but I can provide my doc's info if you want.

I am covered through HAP. I emailed my doctor in Canton asking him about physicals. He replied, "Okay to use Henry Ford name for referral."

I went looking for a DOT physical place but I learned the key phrase is "sports physical." I went here: http://www.miexpresscare.com/sports-camp-physicals/ but you could probably google up a place like that everywhere.

Doug, that's interesting. I have an appointment with my doctor a week from tomorrow. I'll email them the physical and see if there's going to be a problem. Located 20 minutes outside of Livonia, I'll send you a PM with the information once they confirm they'll sign off.

How much do your doctors usually charge for a physical?

Sent my Doctors the form, they said they'd do it.

15 minutes from Livonia, 30 minutes from the track.

Hi guys.

Thanks for the input. I did find a place on Middlebelt RD. and 8 Mile. They still looked at my funny but they did the physical. Really all they wanted to know is where is this racetrack and how can they get in to check it out. So I gave them a ticket to Waterford Hills and directed them to the web site.

So I passed.

8)

Thanks again
